Astara Wellness provides community nursing care for eligible NDIS participants who require disability-related nursing supports in the home or community.

Community nursing care allows participants with complex or ongoing health needs to receive professional clinical support in their own home or community setting, rather than in a clinical environment. This approach supports comfort, dignity and independence - keeping you in the place you know best.
Our community nurses are AHPRA-registered professionals who work collaboratively with participants, their families, GPs and other healthcare providers to deliver safe, coordinated, person-centred care. We treat the whole person, not just the clinical need.
All care is delivered in strict accordance with best-practice clinical guidelines and NDIS quality and safeguards requirements, with comprehensive documentation and regular review of care plans to ensure they reflect your current needs and goals.
What this support can include
Depending on the participant’s assessed needs and care plan, community nursing support may include:
Health assessments and nursing care planning
Medication support within clinical scope
Wound care and dressing support
Continence-related nursing support
Diabetes-related support where appropriate
Catheter care where clinically required
Bowel care support where appropriate
PEG feeding support where required
Respiratory or complex care support within scope
Training and guidance for support workers where suitable
Monitoring changes in health and wellbeing
Liaison with GPs, specialists and allied health professionals where required

For many clinical supports, a referral or care plan from your GP or treating specialist is required. Our team will work with you to ensure all necessary clinical documentation is in place before care begins.

Yes. All nursing staff delivering community nursing care through Astara Wellness are registered with the Australian Health Practitioner Regulation Agency (AHPRA) and hold current registration as Registered Nurses.

Absolutely. Community nursing care is often most effective when coordinated with personal activities assistance or daily tasks support. We work with your full care team to ensure a joined-up approach.

Visit frequency depends on your clinical needs and NDIS plan funding. Some participants need daily nursing visits; others weekly. We develop a schedule in consultation with you and your treating team.
