Life Stage & Transition Assistance
Support through change,
transition & new stages of life
Life changes can be difficult to manage without the right support. Astara Wellness provides assistance to help NDIS participants navigate important life stages, transitions and changes in their support needs.
Making life changes feel more manageable
Life is full of transitions and with the right support, each one becomes an opportunity rather than a challenge. Our Life Stage & Transition Assistance supports are designed to help participants navigate significant life changes with confidence, clarity and dignity.
Whether you're moving from school into adulthood, transitioning into a new home, exploring employment or facing an unexpected life change, our team works closely with you, your family and other key supports to understand what matters most and what kind of support will make the biggest difference.
We don't just plan the transition, we are there through it, adapting our support as your needs evolve and celebrating every step forward along the way.

Frequently asked questions
-
Life Stage & Transition Assistance is available to NDIS participants of all ages, though it is particularly valuable for young people aged 15–25 transitioning out of school. It is equally relevant for adults navigating significant life changes at any stage.
-
Yes - transition support must be funded in your NDIS plan. If it is not currently included but you feel you need it, speak with your support coordinator about requesting it at your next plan review.
-
Absolutely. We encourage family members, carers and other key supports to be involved - with the participant's consent. Transitions affect everyone, and we work best when the whole team is aligned.
-
The duration depends entirely on the nature and complexity of the transition. Some participants need support for a few months; others for a year or more. We adjust the intensity of support based on where you are in your journey.
